We are looking for a Case Manager to enhance the quality of client management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for full client care. Case Manager duties include record-keeping, advocacy, and service coordination for consumers (adults with varying degrees of learning. behavioral, emotional, and developmental disabilities).

RESPONSIBILITIES:

  • Complete Consumer’s quarterly Progress Reports.
  • Complete Consumer’s Special Incident Reports (SIR)
  • Complete case notes.
  • Complete staff notes.
  • Complete Consumer’s Person-Centered Plans.
  • Complete assessments for clients as needed.
  • Complete Monthly staff Schedules.
  • Show up and respond to consumer residences during behavior episodes as needed.
  • Sit-in for a shift if unable to find staff coverage.
  • Regular and reliable attendance is an essential function of this position.
  • Perform other duties as assigned.

REQUIREMENTS AND SKILLS:

  • Bachelor's degree – or equivalent
  • Two years working with individuals who have special needs in a direct client service capacity or one year as a Case Manager in a social service environment.
  • Knowledge/Abilities/Skills:
  • Excellent verbal and written communication skills and basic math skills.
  • Ability to work on multiple projects simultaneously including the organization, assessment, and solution of the projects.
  • Computer literate (MS Word and Excel).
  • Detail oriented with a high degree of accuracy.
  • Valid California Driver’s License, reliable transportation, evidence of car insurance, and insurable driving record. May be required to obtain a Class B driver’s license within the first 3 months of employment.
  • Pass a DOJ background investigation.
  • Provide evidence of a negative TB test initially and every three years thereafter.
  • Obtain Medication Administration, and First Aid and CPR Certification within the first 3 months of employment and maintain as required.

Job Type: Full-time

Pay: $22.00 - $25.00 per hour
