Position Overview:

Staff Pharmacists ensure timely and accurate preparation and distribution of pharmaceutical products within Sutter Lakeside Hospital. This position assures that the medication orders are consistent with current medical practices, F.D.A. and drug manufacturer recommendations, current literature and/or other information sources. Variations from the above and drug manufacturer recommendations, current literature and/or other information sources. Variations from the above standards must be resolved within the Medical Staff Organization in accordance with established policies and procedures. Staff Pharmacists provide information and guidance in the proper administration of medications, on the request of other health care providers.

Job Description:

These Principal Accountabilities, Requirements and Qualifications are not exhaustive, but are merely the most descriptive of the current job. JOB ACCOUNTABILITIES

Dispensing/Distribution.
• Assures accuracy in preparation of products, records, labels and documentation in accordance with State and Federal rules and regulations and hospital policy.
• Makes effective and productive use of time and resources.
• Provides coordination of dispensing and distribution activities with nursing and provides timely follow through.
• Initiates appropriate formulary policy procedure actions.
• Assists inventory management to assure adequate stocks are on hand.
• Handles STAT and emergency requests effectively and in a timely manner.
• Assures sterile technique is applied by staff in appropriate procedures.
• Demonstrates satisfactory performance and adaptability in all practice areas.


Leadership, Direction and Problem Solving.
• Demonstrates leadership and direction of the staff in establishing priorities and organizing work.
• Assures effective and efficient management of resources by the staff.
• Accepts direction from others.
• Applies problem resolution and intervention processes effectively.
• Refers problems to a higher level authority in an appropriate and timely manner.
• Assures judicious utilization of flex personnel time to match current work load.
• Maintains appropriate and timely communication with staff members, other departments, medical staff, patients and others to assure patient care objectives are achieved.
• Applies appropriate safety and security procedures in the control of hospital and personal property.
• Assures legal and policy limitations on activities of unlicensed staff are uniformly applied.


Clinical Services.
• Provides specific evaluation and follow-up of medication orders and initiates corrective actions when necessary.
• Participates in planning, coordination and performance of patient education.
• Contributes to case evaluation, including chart reviews, Drug Utilization Evaluation (DUE) audits, patient care rounds, and interdisciplinary team activities.
• Provides two professional education inservice presentations or articles annually.
• Effectively utilized drug information resources in providing patient care.
• Provides formulary assistance to Pharmacy and Therapeutics Committee, input and product recommendation.
• Documents clinical service activities in accordance with established procedures (i.e., TDM's).
• Demonstrates knowledge and awareness of current pharmacy practice.
• Participates in investigational drug reviews as presented.
• Participates on the Code Blue Team.


Performance, Communications and Interpersonal Relations.
• Enhances professional growth through participation in appropriate education programs and professional association involvement.
• Demonstrates effective written and verbal communication skills.
• Manages challenging situations effectively.
• Presents constructive ideas and suggestions.
• Cooperates with and respects co-workers, medical staff, nursing and public.
• Assures confidentiality of information.

Quality Assurance and Control.
• Quality Assurance:
• Assesses the appropriateness of medications, orders and initiates problem analysis and resolution processes.
• Facilitates Q.A.R. documentation, analysis and correction of issue.
• Contributes to audit, utilization and review process.
• Quality Control:
• Assures maintenance of proper storage and dating of medications.
• Assures timely completion of floor stock checks in accordance with procedures.
• Assures proper preparation, documentation and disposal of chemotherapeutic agents.
• Assures accuracy and timeliness in preparation of medication records with identification of the accountable pharmacist for all transactions.

. Attends and actively participates in department and facility meetings and classes, including annual fire and safety and disaster programs.
. Follows safety procedures, operates equipment and performs job related duties in a safe manner which prevents accidents from occurring.
. Observes the departmental dress and grooming code standards and consistently wears facility identification badge.
D. Attendance and Reliability
. Sick or absent time off does not exceed the facility guidelines as outlined in Personnel Policy manual
. Provides proper notification of absent and tardiness within the established departmental time frames.



EDUCATION
Equivalent experience will be accepted in lieu of the required degree or diploma.

Bachelor's: Bachelor of Science Degree from a recognized pharmacy school.

Other: Doctor of Pharmacy Degree from a recognized pharmacy school.


CERTIFICATION & LICENSURE

PHARMD-Licensed Pharmacist


PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

Prior hospital pharmacy experience is preferred.

Residency or fellowship experience is desired.


SKILLS AND KNOWLEDGE

Drug Utilization Review procedures, Therapeutic Drug Monitoring is required.

Computer knowledge is desired.

PC programs, Pharmacokinetics, interpersonal and communication skills.

Pay Range is $81.92 to $90.78 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
