JOB SUMMARY:
Swope Health is seeking Physician Assistant and/or Nurse Practitioner to provide medical services as a Float Provider. Professional candidate provides general healthcare assessment, treatment and follow-up commensurate with clinical skills and complying with clinical guidelines, administrative policies/procedures of Swope Health, State of Missouri & State of Kansas and accrediting/funding agencies.
PRIMARY FUNCTIONS:

Conduct health assessments on adult patients (and pediatric patients if within scope of practice) with both acute and chronic medical conditions.

Develop impression of diagnosis and a treatment plan in collaboration with physician.

Conduct health education/counseling/prevention of individual patients/groups at Swope Health.

Participate in interdisciplinary patient staffings.
-
Coordinate/collaborate with Swope Health departments and community service organizations as required by funders, Swope Health and supervisors.

Responsible for integrating healthcare and health education in the Continuum of Care process.

Order/perform and interpret routine laboratory tests.

Develop a health promotion and maintenance plan, including disease prevention, health education and counseling.

Provide care for selected complicated patients as directed and supervised by department physician.

Arrange referrals as needed to other members of the health care team in accordance with SH policies/procedures and/or third party payers.

Attends and participates in professional staff and committee meetings.

Documents complete findings in EMR (Electronic Medical Record) in a timely fashion and completes/locks notes within the specified amount of time.

Participates in on-call after-hours rotation with other providers.

Consistently achieves productivity goals by seeing a combination of scheduled and unscheduled pts.

Submits the specified amount of pt. records to collaborating physician for review.

Coordinate/collaborate with other SHS Depts. prn.

Demonstrate compliance with continuing education requirements for licensure/certification in a timely manner.

Maintain pt. confidentiality at all times including maintaining computer security.

Complete all trainings required by SH within required timeframe.

Be available to work clinic hours as assigned by SH including some evenings and occasionally Saturday clinics.

Provide high quality patient satisfaction.

Provide care for both scheduled and unscheduled pts. prn.

Will operate as a team player, realizing and respecting each team member's role/importance in providing positive treatment outcomes.

Must complete Patient Safety Training curriculum.

Other duties as assigned by supervisor(s).
QUALIFICATIONS FOR NURSE PRACTITIONER:
- Registered Nurse in the State of Missouri with ARNP License
- Master of Science in Nursing Degree
- Spanish and English highly preferred
- Completion of a recognized nurse practitioner program in Family Medicine/Adult Medicine in a university based or university affiliated setting
- Ability to work in a culturally diverse population.
- Able to function as part of a team.
QUALIFICATIONS FOR PHYSICIAN ASSISTANT:
- Must be a graduate of an accredited Physician Assistant program
- Must have a current Physician Assistant license in the State of Missouri
- Certified as a Physician Assistant by NCCPA
- Spanish and English highly preferred
- Ability to work in a culturally diverse population.
- Able to function as part of a team.
