Hours: LVN or RN: (32 hours/week), Monday-Friday, between the hours of 8:00AM-4:30PM.
GENERAL STATEMENT OF RESPONSIBILITIES:
The MAT Nurse serves as a liaison between the Medical and Kumeyaay Family Services departments to coordinate the healthcare needs of opioid use disorder (OUD) patients to assure appropriate, timely level of care and service. Manages substance use disorders by offering/prescribing medications in addiction treatment (MAT) and offers transitional treatment of primary care conditions, within the scope of credentialed privileges and standardized procedures, utilizing trauma-informed, motivational interviewing skills and encouraging patient activation and self-management. Works collaboratively with community partners and outpatient treatment programs and primary care and mental health providers. Ensures appropriate follow-up for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.) and medical hospitalizations and ER visits. Responds to patient and community partner requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner. Able to conduct mandatory survey as provided by the funding agency and submit reports accordingly.
SPECIFIC DUTIES AND RESPONSIBILITIES:
- Coordinates MAT program delivery:
- Works closely with the providers during their process to retain and become trained, qualified, and approved for MAT services for the community.
- Collects data necessary for program evaluation and reviews referrals for members receiving MAT services.
- Coordinates and consults with the Interdisciplinary Team representing the opioid user population and the Program Evaluator consultant.
- Design and develop a process of identifying MAT patients in need of ongoing monitoring, tracking and preventive health screening.
- Manage and follow up with incomplete lab and diagnostic orders for MAT patients.
- Maintains current and accurate records and treatment notes as required by policy.
- Referrals to mental health therapist with follow-up and monitoring of progress via interdepartmental meetings.
- Coordinates and collaborates activities throughout SIHC monthly and annually to fulfill MAT program outreach prevention and intervention needs for the community emphasizing the opioid concerns, at-risk population, and treatment options.
- Coordinates and consults with other MAT program staff to analyze, evaluate and implement clinic workflow processes to support ongoing care needs, recovery services and plans of relapse prevention.
- Collaborates with patient care teams to ensure patients are seen expeditiously in the clinic setting, which may include taking non-MAT related history, such as vital signs, HPI and other patient-care related tasks.
- Collaborate with Quality Management, as needed, to identify areas of quality improvement for the MAT program.
- Ability to work independently, supervise when needed, and follow the terms of the awarding agency.
- Ability to work a flexible schedule may be required based on need.
- Other duties as assigned including RN triage, or additional projects to benefit the clinic team.
QUALIFICATIONS:
Education/Experience: Education, training and/or experience which, clearly demonstrates possession of the knowledge and skills stated above. LVN licensed nurse or RN with MAT experience preferred. Addictions/MAT RN preferred or psych nursing background preferred.
Windows platforms and electronic health record database experience required. Must have knowledge and understanding of HIPAA in the ambulatory care setting with absolute ability to maintain confidentiality.
Licenses/Certifications: Certification in addictions nursing preferred. A valid California driver’s license required with application submission and must be maintained throughout employment. Current California applicable nursing license. Current certifications and/or licenses appropriate to the position’s required education and profession.

Must be responsible and able to exercise good judgment, accept administrative supervision, pay attention to detail, follow instructions, including the ability to interact effectively and communicate with people in a professional and courteous manner. Must be highly confidential and work as a team with other staff. Applicant should be sensitive to client’s needs.
Other: Applicants must successfully pass a pre-screening, including a tuberculin skin test or x-ray and a blood/urine drug screening test. Health must be adequate to perform all duties of the position. Applicant must complete SIHC Application and Authorization Form and submit both to Human Resources prior to the close date indicated.
Physical and Personal Requirements: Normal clinic/office environment. Sit or stand for long periods of time. Reach, bend, climb, stoop, lift up to 25 lbs. repetitive hand movement; use and view PC. Reliable transportation and car insurance as required by the state. Travel as needed.
INDIAN PREFERENCE: In the filling of any SIHC job vacancy, preference may be given to qualified Native American Indians, pursuant to the Indian Preference Hiring Act, 25 USC 472, unless other laws require the filling of a vacancy without regard to Indian preference (e.g. Public Law (P.L.) 94-437, “Indian Health Care Improvement Act” (IHCIA). To receive Indian preference for any SIHC position, the applicant must be enrolled, or be eligible for enrollment, as an American Indian with their Tribe, or must be certified as an American Indian from the designated Tribal Representative. Applicants claiming Indian preference must attach verification of their claim to the SIHC application, including certification of form BIA-4432, which is available from the SIHC Human Resources Department. If verification is not or cannot be verified, the applicant will not receive Indian preference for purposes of the interview.
