We have an opening for an Assistant Care Coordinator who can answer yes to these questions. We are seeking an Assistant Care Coordinator to join our independently owned and operated national agency serving clients from New Haven to St. Albans.

We are scheduling interviews immediately for the Assistant Care Coordinator position.

Assistant Care Coordinator Benefits:

Assistant Care Coordinator Responsibilities:

• 
  Implementing retention efforts to encourage a positive work environment

• 
  Communicating regularly with caregivers, clients and their families
• 
  Handling incoming client inquiry calls
• 
  Completing home assessments for new clients
• Direct Client Care on an as needed basis, including some weekends.

• 
  Providing exemplary customer service

• Interviewing and completing orientation for employee hiring process.
• On Call phone coverage weekend and evenings required (at premium pay when called to action!)
• Licensed to drive and access to a personal vehicle during the normal work day is a MUST.

Assistant Care Coordinator Requirements:

• 
  Experience – Two years experience working with the elder/disabled population preferred

• 
  Education – High school diploma
• 
  Skills – Developing personal relationships, time management skills, exemplary communication skills

• 
  Characteristics – This is an autonomous position, so we’re looking for individuals that are self-motivated and willing to learn.

• 
  Driver License and Reliable vehicle - YOU MUST be able to drive to/from clients homes within our service area (Canadian border to Middlebury and everywhere in between- mileage reimbursement available).
• Weekend availability as needed is an expectation in this position. -Client coverage as needed will be paid at a premium rate when called to cover shift call outs.
