SUMMARY
The Lead Case Manager – TLP/MGH/SH leads the other case managers and interns in the TLP/MGH/SH programs in their day to day duties, makes sure that charts remain in good order, stays up-to-date on productivity expectations, and seeks and provides clinical support under the director of the Clinical Director for Child Youth and Family Services and the Clinical Manager.
The Lead Case Manager is also responsible for carrying a case management case load and providing quality client care by linking clients with community resources, advocating in meeting client needs and completing documentation. This position takes clients who are in Synergy House, the Transitional Living and Maternity Group Home Programs and community youth on occasion. The Case Management activities will vary by program and client need. This position follows the site SOPs in all day to day operations.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Provides direct day-to-day direction to other case managers and case management interns and meets with them individually for one-on-one assistance.
Completes administrative duties as required including, but not limited to, chart reviews, data collections, reports, serving on committees, etc.
Communicates openly and directly with the Clinical Director and Clinical Manager to offer clinical case support to all case managers to receive personal supervision.
Acts as a liaison between clients and external agencies and other internal Synergy resources. (i.e. medical services, school, external social workers, etc.) Assists clients with telephone calls to resources as requested and/or needed. Completes correspondence as required.
Assists clients, as needed for program, in locating assistance for housing, utilities, and medical services, employment search and seeking affordable daycare. Assists clients with clothing needs for themselves and family members as needed.
As program requires, meets weekly with clients to set goals and monitor client’s progress.
Works to arrange transportation with program to necessary appointments, or provides transportation if necessary.
Facilitates educational groups for clients as required by program.
Assists clients in crisis.
Consults about client behaviors and activities. Intervenes and provides assistance as needed.
Maintains med tech and safe crisis management and CPR certifications.
Completes client assessments and other paperwork as needed. Completes discharge summaries as required. Maintains accurate documentation of client record and maintains files in accordance with Synergy standards. Completes documentation both accurately and professionally in providing necessary information about a client’s stay. Communicates via com log, etc. to ensure advocates have necessary information to meet clients’ needs.
Maintains communication with other staff members – both oral and written.
Attends community/agency case manager meetings as required and relays relevant information to staff.
Attends staff meetings and weekly case review.
Attends internal and external trainings and in-services as budget and time allow.
Maintains client confidentiality.
Acts as the on-call supervisor and makes appropriate decisions as required/needed when serving in the on-call rotation as required.
Participation in Performance and Quality Improvement activity is required by each employee.
Performs other duties as assigned.
SUPERVISORY RESPONSIBILITIES
No direct supervisory responsibilities but will lead/direct day-to-day work of the assigned case managers and interns.

EDUCATION and/or EXPERIENCE
Master's degree in Social Work, Psychology, Counseling or related field from an accredited college or university.
LANGUAGE SKILLS
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS The employee in this position must have a valid Class E Drivers license. The employee in this position must have CPR/First Aid Certification and be a Medical Technician.
OTHER SKILLS AND ABILITIES The employee in this position must have the ability to transport up to fifteen (15) passengers in a van. The employee in this position must have the ability to type and perform Data Entry/10 key, and use computer software, including Windows, Word for Windows, Alice, and Access.

Bachelor's degree in Social Work, Psychology, Counseling or related field from an accredited college or university.
All staff must be 21 years of age, pass a background check (Children’s Division/Central Registry/Child Abuse), drug screen, fingerprint screen, have a valid driver’s license and proof of insurance. Some evening hours may be required.
