Job Description:
The Full Service Partnership Program Team will provide intensive services to adults with serious mental illness at risk of acute or long term institutional care. The overall goal of the program is to help adults with severe and persistent mental illness succeed in the community with sufficient structure and support. The Case Manager will provide the full range of trauma informed services including individualized planning, case management, integrated co-occurring drug and alcohol services, medication support, linkage to housing, integrated vocational services, and access to support on a 24/7 basis. The TMHA Case Manager and Clinician work as a team to manage a caseload of 15-20 adults with serious mental illness. The Case Manager will be responsible for crisis care, case management service delivery, individual rehabilitation activities, day to day skills building, transportation to shopping and appointments and all other aspects required to help the clients maintain activities of daily living and stability.
