Position Summary: Manages and coordinates all aspects of patient care in accordance with State and Federal rules and regulations and the nursing standards based on the scope of a licensed independent practitioner.
Job Responsibilities:
Manages a clients overall care; diagnoses health problems; prescribing medications and treatments within the scope and formulary set forth by collaborating physician; treat chronic and acute conditions including injuries, infections, diabetes and high blood pressure; detect changes in patient condition and recommend further treatment options or future appointments if needed; provide after-care / ongoing care information and resources to patients
Perform initial patient screenings, patient history and physical examinations; facilitate new patient consultation; perform invasive procedures; ensures patient records are accurate at each patient appointment; administer medications; operate medical equipment
Order, conduct, analyze and interpret diagnostic test such as x-rays, EKGs and lab work
Deliver quality, physician-level care while complying with agency policies, safety regulations, and productivity metrics
Ensure patient records are in compliance and up to date with accurate and current test results, immunizations, condition changes
Collaborate and consult with other healthcare professionals and multidisciplinary team members as needed; provide referrals to other practitioners when necessary
Serve as a consultant and resource for patients need for ongoing care information; provide counseling and guidance for any patients with critical care conditions; facilities staff, patient and family decision making by providing educational tools; acts as patients advocate for families, individuals, groups and communities; educate patients about preventive care and prescribed treatments
Facilitates the patients transition within and between health care settings (admitting, transferring and discharging patients)
Provide oversight, delegation and direction to RN, LPN and MA; ensure use of established procedures, policies, guidelines and standards under the disciplines scope of practice

Required Knowledge, Skills and Abilities:
Excellent written and verbal communication skills

Proficient in medical instruments and equipment
Familiarity with the EHR medical charting system

Job Requirements:
Masters Degree in Nursing
Prescriptive authorization
Minimum 5 years nursing experience
Active Nurse Practitioner license in Ohio
Valid drivers license and current auto insurance

Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
