DISCHARGE PLANNER/CONTINUING CARE COORD

Responsibilities:
At Talbott Recovery Campus we focus on patient-centered care and an individualized approach, upholding a high standard of innovative treatment and compassionate attention to each patient's needs. We are committed to providing and promoting evidence-based treatment for individuals with co-occurring disorders through clinical services, education, and research. Our purpose is to create lifetime relationships for long-term recovery. Everything we do is designed to support recovery in a comprehensive way, addressing the needs of the whole person, physically, emotionally, mentally, and spiritually.

We are currently hiring a Discharge Planner/Continuing Care Coordinator, responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients.

This is a full time position with generous benefits including medical, dental, vision, 401(k) match and PTO.

Qualifications:
This position requires the ability to establish and maintain referral relationships by coordinating all communication and information exchange within the scope and boundaries of Talbott policies.
The Continuing Care Coordinator develops a comprehensive and patient-focused continuing care plan for assigned patients.

Experience in alcohol/substance abuse treatment with emphasis on the 12-Step program, experience in leading groups and writing treatment plans.

Master's Degree or CADC preferred.
