Our Southeastern Massachusetts Adult Services Program is seeking a 30 hour per week RN or LPN for our residential program on Cape Cod Ma Schedule is generally Monday through Friday but requires flexibility as is based on the needs of our individuals Responsibilities Obtains and maintains knowledge of all individuals including significant medical history,
current medical problems, current medications, usual behavior and appearance and
report changes as necessary and appropriate Conducts routine and problem-centered health assessments. Also develops and implements appropriate interventions Provides consultation for hospitalizations to include: admission visits, discharge planning and follow up within the next business day of return home Completes daily, monthly, quarterly, and annual nursing assessments notes as needed. Performs periodic reviews of the medication record to ensure accuracy and review all medication occurrence reports. Provides staff training for pertinent medical concerns. Administers medical treatment prescribed by the individual’s healthcare provider as per regulations Delivers first aid in emergency situations, as necessary Attend meetings as necessary Provides telephone consultation services during shift and after work hours and has on call weekend
