A Long Island-based health services network is currently seeking a Licensed Practical Nurse (LPN) to join one of their Adult Residential facilities in Suffolk County.
About the Opportunity:
- Setting: Residential Facility - ICF
- Schedule: Monday to Friday
- Hours: 7:30am to 4pm (Flexible)
- EMR: Epic
- Caseload: 12 to 15
Responsibilities :
The Adult Residential LPN will:
- Participate in written nursing plans of care for caseload with RN Supervisor
- Maintain medical record for caseload
- Ensure all medications are administered and stored as necessary
- Prepare and review monthly MAR's and medications
- Schedule and review all medical consults
- Ensure medical follow through on all medical appointments, including any new orders and follow appointments needed
- Act as a liaison with the Pharmacy team in ensuring medications are provided to residence
- Review and complete incident reporting paperwork, as required
- Ensure compliance with OPWDD regulations
- Perform other duties, as needed
Qualifications:
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN)
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bachelor's Degree
- Knowledge of OPWDD regulations
- Experience with ID/DD
