Hospice RN Case Manager
Job Posting for Hospice RN Case Manager at Tapestry Hospice
An RN Case Manager coordinates individual patient care activities as a key member of the interdisciplinary team and provides holistic nursing care to terminally ill patients and their families, including appropriate support for families at time of death. Nursing care is based on the nursing process, consisting of patient/family assessments, diagnosis of key problems, the development of a plan of care, implementation of interventions, patient education and evaluation of patient outcomes. ***(Must live in one of the following counties: Fannin, Union or Gilmer).***
Requirements:
- Current Registered Nurse License.
- A self-driven individual with a desire to deliver excellent customer service.
- Excellent communication skills, verbal and written.
- Excellent clinical assessment skills.
- Computer skills (EMT, email, and familiarity with health care applications).
- Excellent time management and supervision skills.
- Ability to accurately document a patient's current status.
- Advanced knowledge of pain management & symptom control.
- Valid Driver's License and reliable transportation
Preferred Experience:
- Hospice Case Management
- Critical Care, Surgery or Oncology
