RETENTION PAY ($$$) AVAILABLE FOR TARRANT COUNTY EMPLOYEES
Full-time employees: May be eligible to earn up to 2 retention payments in 2022 totaling $3,900 ($1,000 on September 16th and $2,900 on December 9th.) Note: Employees must be employed on each payment date to be eligible.
Part-time and Substitute employees: May be eligible to earn up to 2 retention payments in 2022 totaling $750 ($250 on September 16th and $500 on December 9th). Note: To be eligible, part-time and substitute employees must work an average of 16 hours in specific months preceding the payment dates and be employed on each payment date to be eligible.

- Coordinates referrals with social service agencies in and out of the Ryan White continuum of care.
- Develops and maintains relationships with community partners to facilitate referrals.
- Performs and documents initial patient interviews as assigned.
- Documents all clinic and ADAP eligibility screens and services provided in required databases.
- Receives and documents referrals from clinical providers. Develops a plan of care related to referrals before patient leaves the clinic if possible. Facilitates appointments.
- Follows up and documents monthly "lost to care" activities.
- Provides transportation support to eligible patients. Documents and tracks all transportation assistance tools.
- Maintains clinic list of all ADAP eligible patients. Performs initial ADAP eligibility screening and recertification every six months thereafter.
- Works closely with medical and non-medical case managers in care team to ensure well-coordinated medical care.
- Attends ADAP trainings annually.
- Performs all other duties as assigned.
Monday, Wednesday, Thursday & Friday 8 am - 5 pm and Tuesday 10 am - 7 pm
- High school diploma or GED.
- One (1) year working in a social service or healthcare setting.
- Experience with basic health education preferred.
- Experience with HIV/AIDS preferred.
- Bilingual in English and Spanish (preferred is a plus, but not required).
- Good organizational and time management skills.
- Strong interpersonal and communication skills.
- Detail oriented with the ability to follow-up with patients and services providers in a timely manner.
- Possess outstanding customer service skills, especially with patients.
- Basic computer and phone skills.
- Ability to focus and complete tasks.
