Full time Transitional Housing Case Manager needed for an exciting opportunity to work as part of a multidisciplinary team of case managers and clinicians providing transitional housing case management for individuals living within the Los Angeles County who are living with HIV/AIDS. Job duties include assisting clients with transitioning back into the community by helping with referrals to medical providers, housing services, community care providers, connecting clients to substance use treatment facilities and providing support and education. Bilingual preferred

Requirements:

  • SUD Counselor Certification
  • California Driver License and Insurance
