Tarzana Treatment Centers is hiring a Registered Nurse for a Diabetes team that will work closely with patients in Mental Health and Substance Use Disorder treatment that are at risk or are diagnosed with Diabetes. Hired applicant will work closely with Case Management Team and support with treatment and transitions of care. This position will work with a team of Behavioral Health Coordinators who also work with Diabetic patients. This specialized team will consist of Case Managers, Medical Assistants and/or LVN's, and will work parallel with Medications for Addiction Treatment (MAT) team members. 

Ideal candidate will have experience and/or knowledge of SUD, Diabetes, Risk of Diabetes, and also Behavioral Health issues. Candidate must support MAT services and the belief of MAT medications if needed and recommended. Ideal candidate should also know comorbidity regarding Diabetes and Alcoholism, Diabetes and Depression etc. 

Candidate must be a Registered Nurse with the State of California, and have a High School Diploma or GED. License must be current upon hire. Those in Recovery are encouraged to apply!
