Butts County Counseling Center - Case Manager M-F Full-time days

Work as a member of an outpatient treatment team to provide community - based or home based case management and psychosocial rehabilitation services to individuals with mental health diagnoses. Will provide skills training, education, safety plans and care coordination. Will also work with other community agencies and family to assist in the individuals recovery. Complete electronic documentation daily.

NOTE: McIntosh Trail Community Service Board conducts background checks on all final candidates. Hiring of applicants is contingent upon satisfactory results of:

Fingerprint Based Background Check of the Georgia and Federal Criminal History Record 
Database
Drug Screen
Motor Vehicle Report (MVR) for seven years

TB testing will be required upon appointment. If you have previously tested positive for TB, you will be required to bring a copy of your chest x-ray on your first day of orientation.
