We are looking for a part time (2-3 days/week) Nurse Practitioner to join our dynamic team in our Assisted Living Facility in the Port Jervis, NY area!

Job Qualifications:
- Master's or Doctorate degree in Nursing (MSN or DNP) from an accredited program.
- Current and unrestricted Nurse Practitioner license in NY
- Certification in Adult, Family, or Rehab specialty from a nationally recognized certifying body.
- Minimum of 1 year of experience as a Nurse Practitioner, preferably in adult, family, or rehab settings.
- Experience working in an assisted living facility or similar long-term care environment is a plus.
- Solid understanding of geriatric care principles and best practices.
- Excellent communication and interpersonal skills, with the ability to work effectively within a team.
- Strong critical thinking and problem-solving abilities to address complex medical situations.
- Proven ability to manage time efficiently and prioritize tasks effectively.
- Willingness to embrace our facility's mission and values, promoting a culture of compassion and respect.
Responsibilities include but are not limited to:
- Conduct comprehensive health assessments for adult residents with a focus on adult, family, or rehab specialties.
- Collaborate with the interdisciplinary team to develop individualized care plans and treatment goals for residents.
- Monitor residents' health status, evaluate responses to treatments, and make necessary adjustments to care plans.
- Prescribe medications
- Perform minor medical procedures and wound care as needed.
- Educate residents and their families about medical conditions, treatment plans, and self-management strategies.
- Provide support and guidance to nursing staff in managing complex medical cases and resident care issues.
- Assist in coordinating and conducting staff training on relevant medical topics and best practices.
- Collaborate with external healthcare providers, specialists, and hospitals as needed.
- Participate in facility-wide quality improvement initiatives to enhance resident care and safety.
- Adhere to all legal and ethical standards of nursing practice, ensuring confidentiality and privacy of resident information.
- Maintain accurate and up-to-date documentation of all patient encounters, treatments, and medical records.
- Foster a positive and caring environment for residents, families, and staff.
