Flagstaff area state long-term care facility is seeking a Licensed Social Worker to join our team. Responsibilities:
- Oversee and provide medically related social services to maintain the well-being of residents.
- Maintain a personal case load of assigned patients.
- Ensure compliance with federal and state regulations as well as facility policies.
- Implement and monitor the Arizona Management System (AMS) methods.
- Utilize AMS concepts and tools for problem-solving, process improvement, and work efficiency.
- Foster a patient-centered care approach with a cultural focus.

- Case management proficiency.
- Resident assessment capabilities.

Preferred Skills:
- Previous experience in a healthcare setting working directly with individuals.
- Long-term care experience.
- One year supervisory experience
Education Requirements:
- Active social work license issued by the Arizona Board of Behavioral Health Examiners.
- Ability to obtain and maintain a valid AZ DPS Fingerprint Clearance Card.
- Valid state-issued driver's license.
- Compliance with CMS Interim Final Rule vaccination requirements.
- Current Negative TB skin test or a physician's statement indicating freedom from tuberculosis if previously positive.
