Responsibilities:
- Provides a comprehensive speech therapy evaluation.
- Documents findings on the standardized evaluation form neatly, accurately, and adhering to all guidelines.
- Provides a comprehensive treatment plan including long and short-term goals, frequency, duration, and treatment modalities.
- Ensures MD orders are obtained for evaluations, treatments, recertifications, and discharges.
- Provides comprehensive treatment to patients utilizing modalities and modifies patient treatment as indicated while adhering to precautions.
- Speech Language Pathologist – CFY completes all documentation as necessary.
Qualifications:
- Graduate of an accredited university with an MA, or M.S. in communication disorders that is recognized by ASHA
- Holds current license and/or registration in the state (as applicable)
