Responsibilities

This position provides outreach and community engagement for the non-title (NT) 19 - Serious Mental Illness (SMI) population. Terros Health has recently been awarded a unique opportunity to serve under-insured, non-title 19 (non-Medicaid) individuals with SMI or COD in Maricopa County through the Community Mental Health Centers (CMHC) grant program. The Community Outreach Specialist’s population of focus is to outreach NT 19 SMI patients experiencing homelessness and those with a substance use and/or co-occurring disorder.

Starting at $17.18/Hour D.O.E.

$2,000 sign on bonus! 

Full-Time Employed: 40 hours/week

Location: 51st Ave and Camelback Rd. in Phoenix, AZ 

Schedule: Monday - Friday (some flexibility is required) 

Duties include:

• Work as a team member with other personnel and collaborators to promote interventions to improve health outcomes related to substance use, co-occurring disorders, and homelessness.
• Focus on SMI patients experiencing homelessness and those with a substance use and/or co-occurring disorder who are NT 19 and enrolled in the CMHC grant.
• Conduct community outreach throughout Maricopa County each week to identify and locate SMI patients who are non-complaint with the clinical team and homeless.
• Provides Intensive Homeless Street Outreach with specific work with the SMI population.
• Act as a point of contact for the department by responding to inquiries and requests or directing as appropriate and providing information regarding programs, courses, policies, and procedures to internal and external contacts.
• Making connections with outside stakeholders within the broader community.
• Cultivating relationships with other agencies and business that also support the Homeless Population.
• Provide resources, hygiene packages, assist with securing personal documents such as ID
• Fostering new relationships and collaborations at different Valley Shelters

Qualifications

• • GED/High school diploma required 
  • Minimum of one-year work experience and/or lived experience within the field of homelessness, substance use and or mental health field.
  • Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
  • Schedule flexibility and ability to work evenings if the program management deems it necessary to achieve the objectives of the program.
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy

  • Must have a valid Arizona Level 1 Fingerprint Fingerprint Clearance card or be able to apply for one within 7 working days of employment

  • Must pass background check, TB test and other pre-employment screening
