Job Summary: The Case Manager acts as a coordinator between the sheltered client, shelter facilities and resources. They will provide case management support to clients which may include documentation support, federal program eligibility, grant applications, transition housing, case management support, and resource provision.
Key Responsibilities:
- Client Care: Call clients, as needed, to facilitate document exchange, online or phone follow up, and general social service engagement.
- Reporting: Maintain accurate records of shelter activities, client needs and incidents. Prepare reports and share essential information.
- Incident Response: Act as a point of contact during incidents, implementing protocols as necessary. Identify social service needs of clients and report to supervisor. Complete incident reports as needed.
- Community Engagement: Foster positive relationships with community partners, local authorities, and other stakeholders to support the shelter's mission.
- Weekend & On-Call Availability: Must be able to work limited hours on the weekend. Remote activity.
- Communication - Serve as a liaison between project team and facility staff.
Qualifications:
- Social work experience.
- Strong organizational and communication skills.
- Ability to maintain professional demeanor during incidents.
- Knowledge of social services resources and support systems.
- Bilingual (preferred).
- Proficient in MS Office.

It is understood that your employment is to be on an "at will" basis. This position with Tetra Tech is temporary and on an "as needed" basis and will end.
