What are the responsibilities and job description for the Preferred Communities Program Manager position at The Alliance?
The Preferred Communities Program Manager is responsible for managing the Preferred Communities (PC) program. The PC Program Manager ensures the PC Case Management team is operating effectively and efficiently within the guidelines of the required PC Services. This would include overseeing social services, supervising staff, resource development, and managing annual plans and budgets. The PC Program Manager is required to utilize problem-solving skills to oversee the delivery of client services by the Case Managers, while sustaining and developing relationships across departments, and within the community.
The PC Program Manager serves as a leader and a resource to educate and promote the professional development of Case Managers to focus on providing quality delivery of services to participating clients and ensure that the services are designed to address the unique needs and strengths of each client. The PC Program Manager will assist with the development, communication, and implementation of methodologies to enhance program effectiveness and provide input to revenue cycle members to align goals and mission.
Responsibilities:
- Develop and implement the philosophies, policies, structure, and goals for the PC program in alignment and coordination with standards provided by ECDC.
- Conduct quality assurance audits and activities to assess the effectiveness of services delivered and ensure documentation in client chart are in compliance.
- In partnership with the Refugee Health & Social Services Director, supervise graduate social work interns
- Complete and evaluate monthly, trimester, and annual reports of the Department’s functions.
- Maintain a working relationship with local, state, and federal agencies, recognizing the agency’s position in the community and its needs for cooperation and assistance for such services.
- Provide consultation to assist the Case Managers with promotion of motivational interviewing, client contributions to their service plan, and reinforcement of self-management skills.
- Collaborate with the Program Coordinator(s) to assign cases to Case Managers.
- Organize and coordinate special health education programs and regular agency health fairs.
- Collaborate with multiple agency programs and staff, and participate in bi-weekly interdepartmental meetings to coordinate agency services.
- Represent the agency and program at internal and external meeting as needed.
- Participate in policy development and continuous quality improvement activities.
- Supervise, train and develop program staff, including case managers, coordinators, support staff and volunteers.
- Supervise graduate social work interns
- Develop and oversee the annual budget of the PC program
- Coordinate trainings, monitoring, and technical assistance for the Houston Refugee Services Case Management Consortium
- Complete and evaluate monthly, trimester and annual reports of the department's functions and programs
- Collaborate internally across departments and externally network with community partners to enhance service provision across SAS, PC and PC for APA
- Develop and manage contacts throughout the community, and in particular with health service providers, such as the University of Texas Medical School and University of Houston School of Social Work in order to collaborate to provide health services to clients receiving services
- Maintain a working relationship with local, state and federal agencies, recognizing the agency's position in the community and its need for cooperation and assistance from such services.
- Find opportunities to expand the Refugee Health and Social Services department through grant writing and seeking other funding sources
- Collaborate with multiple agency programs and staff; participate in weekly interdepartmental meetings to coordinate agency services
- Conduct quality assurance and monitor client case file for RMA eligibility
- Other duties as assigned
Qualifications:
- Master of Social Work and other Social/Human Science degree preferred
- Supervisory experience required
- Medical Social Work experience preferred
- Program management experience required
- Experience working/volunteering with foreign born populations and/or refugees required
- Knowledge of health, public benefit programs and social service resources in the Greater Houston area required
- Foreign language fluency in a language spoken by refugee populations strongly preferred
- High degree of independent judgment, flexibility and problem-solving skills
- Valid driver’s license, automobile liability insurance and knowledge of the city
Salary : $55,000 - $58,000
