Community Living Weekend Coordinator

POSITION SUMMARY:

This position is a professional management role tasked with implementing The Arc Baltimore’s Mission in a manner that embodies the Core Values for and with supported individuals, their families, friends, and other members of their developing social network.

DUTIES & RESPONSIBILITIES:

• 
  Ensure that individuals who receive services are supported in realizing their aspirations and goals.

• 
  Respect the choices of individuals and provide information, encouragement, and guidance in the exercise of those choices.

• 
  Ensure that individuals who receive services have opportunities to develop meaningful relationships, and that they receive assistance, modeling, coaching, and support in doing so.

• 
  Ensure that individuals who receive services have opportunities to develop membership in social groups and communities and receive assistance, modeling, coaching, and support in doing so.

• 
  Ensure that the health and safety needs of individuals who receive services are met consistent with COMAR, Center for Medicaid Services, and Board of Nursing regulations, accreditation standards, and The Arc Baltimore’s policies and procedures.

• 
  Ensure proper reporting of all incidents to QE, family, advocates, guardians and supervisors as required by the Office of Health Care Quality and The Arc Baltimore during weekends, as needed.

• 
  Provide comprehensive weekend report to weekday coordinators, director and VP of Community & Family Living at the end of each weekend.

• 
  Ensure that regularly scheduled and emergency medical services for supported individuals are completed as required and as needed during the weekend.

• 
  Work in collaboration with peers and co-workers to ensure proper oversight of DSP’s and supported individuals during the weekend.

• 
  Participate in hiring process and develop Direct Support Professionals.

• 
  Provide oversight, coaching and training on systems and expectations to DSP’s during the weekend, as needed.

• 
  Participate in bi-weekly coordinator meetings, as scheduled.

• 
  Work cooperatively with intra and extra agency professionals and stakeholders to implement The Arc Baltimore’s mission, including supporting individuals who receive services in their employment aspirations.

• 
  Randomly audit staff progress with using The Arc Baltimore’ database to ensure accurate and timely input of data and information.

• 
  Monitor time keeping of staff for validity and accuracy, in alignment with time keeping policies and The Arc Baltimore time management system.

• 
  Manage staffing and overtime during weekends shifts, as needed.

• 
  Provide on-call coverage on the weekend for emergencies and staffing communications, beginning Friday at 9am and ending at 9am Monday.

• 
  Conduct regular home visits and ensure communication of maintenance and other environmental issues to area director and maintenance department.

• 
  To attend all meetings and trainings as required.

• 
  To fulfill other job duties as assigned.

• 
  To participate in The Arc Baltimore’s committees and work groups.

• 
  To assist with The Arc Baltimore’s fundraising and other events.

EDUCATION & EXPERIENCE:

• 
  A Bachelor’s degree in Social Work, Psychology, Special Education or a related field is required for this position. In lieu of a Bachelor’s degree, five years’ experience as a Direct Support Professional may be substituted.

• 
  One year of case management and supervision experience in the disability field is required for this position. In lieu of such experience, five years’ experience as a Direct Support Professional may be substituted.

• 
  A Bachelor’s degree in Social Work, Psychology, Special Education or a related field is required for this position.

• 
  One year of case management and supervision experience supporting people with disabilities is required for this position.

• 
  A minimum of six months providing direct support to people with disabilities is required for this position.

• 
  Must have and use personal car to conduct agency business, including the transportation of supported individuals.

• 
  Some lifting up to 50 lbs.

• 
  Must be available during scheduled work hours by use of a personal cell phone.

• 
  Must work effectively within a team environment.
