Job Summary:
- The purpose of the Supported Living Coach is to ensure that people receiving Supported Living services maintain an independent living situation of their own choosing in a community setting.
- The Supported Living Coach will provide training and assistance in a variety of activities to support persons who live in their own homes or apartments.
Responsibilities:
- Work in a collaborative effort with program participants, their families, and professional staff.
- Participate in annual ISP and/or other support meetings to help establish goals for each participant.
- Identify consumer’s strengths, weaknesses, barriers, and developmental capabilities.
- Provide individuals with Supported Living training, in order to increase and improve their skills that lead to outcomes specified on their ISP.
- Counsel consumers in a professional manner as required.
- Adhere to proper documentation requirements and furnish reports/documents in the time period specified by the Supported Living Manager.
- Responds to crisis situations for people in Supported Living needing emergency response.
- Transport consumer to necessary appointments.
- Assist consumers with grocery shopping.
- Accompany all consumers to doctors’ appointments and care for all medical needs.
- Responsible for all household bill paying; bank accounts, and any other financial requirements.
- Maintains records and client service plans, participates in Individual Support Plan program planning, service planning and implementation.
- Perform other duties as assigned.
Requirements:
- Must possess a bachelor’s degree from an accredited college or university with a major in nursing, education, or a social, behavioral, or rehabilitative science.
- Candidates without a degree must have two years of college as stated above and two (2) years of experience working with in a medical, psychiatric, nursing, or childcare setting or working with persons who have a developmental disability; or experience in one of the aforementioned fields may substitute for education on a year-for-year basis.
- Must pass a Level II Background Screening.
- Must pass a drug test.
Must have transportation, current license, current car registration, current car insurance, and a good driving record.
