We are seeking an experienced Social Worker to join our team.
Job Description includes but is not limited to:
Leads the inter-disciplinary team in Resident Care Conferences to ensure the social, psychological, and emotional needs of each resident and family member are met.
Oversees appropriate assessment prior to Initial Care Conference according to Minimum Data Set guidelines.
Maintains the ongoing MDS’ assessments and care plans in related areas.
Completes documentation in every resident’s record through care plan reviews and pertinent daily recordings.
Provides direct social work services to residents and family members in the areas of counseling, group work, problem solving, information and referral, financial assistance, legal assistance, and advocacy to facilitate adjustment to the facility.
Carries out social service duties in accordance with current federal, state, and local standards, guidelines and regulations, our established policies, and procedures.
Assures that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
SPECIFIC REQUIREMENTS:
Must be able to read, write, speak, and understand the English language.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and public.
Must have patience, tact, cheerful disposition, and enthusiasm, as well as being willing to handle residents based on whatever maturity level in which they are currently functioning.
Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing Social Service practices.
Must be able to relate information concerning a resident’s condition/incident to the Interdisciplinary Team as appropriate.
EXPERIENCE:
A minimum of one (1) year social work in a hospital, skilled facility, or other health-related field preferred.
EDUCATION:
Must possess, as a minimum, a BSW from an approved school of Social Work.
Proof of COVID Vaccination is required to be eligible for this position.
