Position Overview :
As an Intake Assessor, you'll play a pivotal role in conducting psychosocial assessments, determining appropriate levels of care, and facilitating referrals to specialized programs.
Your responsibilities include intake for individuals seeking services, ensuring accurate and timely documentation.
Key Responsibilities :
- Conducting psychosocial assessments for individuals seeking services at The Crossroads Center.
- Utilizing knowledge of the Diagnostic and Statistical Manual of Mental Disorders and American Society of Addiction Medicine Criteria to determine appropriate levels of care.
- Collaborating with clinical staff to address intake-related clinical challenges.
- Ensuring documentation is completed within agency standards.
- Providing exceptional customer service to both internal and external customers and resolving service issues.
- Assessing risk factors for individuals seeking treatment.
- Ensuring timely and accurate documentation of patient presentations.
- Obtaining prior authorizations for individuals entering care, as needed, to ensure financial viability.
- Maintaining a caseload as required.
- Performing other duties as defined by the organization.
Experience & Qualifications :
- Bachelors degree in social work, counseling, or related field (Masters preferred).
- Minimum of 2 years of intake / assessment experience with substance use and / or mental health disorders.
- Ohio state licensure as an LSW, LISW, LPC, or LPCC.
Physical Requirements :
While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel;
reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and / or crouch.
The employee must occasionally lift and / or move up to 15 pounds). Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
Keyboard data entry required.
