Outreach Coordinator

The Enrollment Casemanager conducts intake visits for waiver applicants for in home care and acts as a facilitator for the eligibility determination process, providing assistance from the time individuals are identified as potential participants in an HCBS program to the time they are enrolled as participants or are terminated from the intake process.
Working with people with disabilities and the elderly. Intake visits are conducted in the client's home and will require travel.
Bachelors degree in Human Services Field
One Year Social Work Experience
Drivers license required.
Job Types: Full-time, Contract
Pay: $20.50 per hour
