This will be a complete hands-on experience and a chance to have meaningful interactions with our Elder Care Navigation clients! You will interact with clients in various ways such as client meetings in the office and in homes/facilities, contacting them for regular check-ins, and more. You will be trained and then responsible for meeting summaries and functional assessments. You're the right applicant if you want to apply your interpersonal skills, are excited to provide essential support to our Elder Care Navigator, learn more about care coordination and enjoy working “directly” with clients. The person we are looking for must exemplify professionalism and understand what it means to be a team player. We believe that every person at our firm is essential to our success.
