Overview

The Clinical Team Manager oversees the delivery of patient/family care across the life span (perinatal, pediatrics, and adults) in a defined geographic region. Applies concepts derived from management and communication theory, group dynamics, coaching/education methodologies and expert clinical judgment to ensure optimum quality of patient care and services.

 

The Elizabeth Hospice is San Diego’s oldest non-profit hospice of 45 years.  We are growing and looking for talented individuals to join our high performing team and support our mission.  We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m. fully in office in Mission Valley, CA

Salary: 93,000-$130,000 annually. Salaried exempt position. Subject to experience and qualifications.

Work Location: San Diego, CA

Travel: Travel required for supervisory patient visits, occasional travel for educational purposes.

 

 

Responsibilities

  • Implements agency strategic plans and protocols to assure compliance with all federal, state, and local health care regulations as they affect the provision of clinical services.
  • Monitors budget variances and is accountable for results.
  • Interviews, hires, reviews, and evaluates the performance of assigned clinical staff.
  • Direct supervision, and evaluation of the clinical practice and performance of the hospice team.
  • Participate in plan of care decisions and keep informed of treatment and management plans for patients and their caregivers.
  • Facilitates IDT.
  • Supports same day admission and registration visits in collaboration with the Access Center leadership
  • Assigns newly admitted patients.
  • Supports the planning, coordination, and delivery of patient/family care in the absence of other Team Managers.
  • Collaborates with Compliance and Quality Assurance Departments to investigate and resolve quality concerns.
  • Monitors utilization of personnel, coordinates follow-up of employees involved in workman’s compensation in collaboration with HR.
  • Participates in agency on call procedures.
  • Collaborates with interdepartmental managers to assess deficiencies and determine process improvement needed.
  • Responsible for project development, implementation, and evaluation as directed.
  • Qualifications

  • Bachelor’s Degree preferred for RN, preferably in nursing, health care administration, or business administration, Masters of Social Work required for MSW; 3 years healthcare management experience with 2 years’ hospice supervisory responsibilities preferred; or equivalent combination of education and experience.
  • Requires knowledge of hospice care, the dying and bereavement process and demonstrated clinical expertise in managing patient and family concerns.
  • Current CA RN licensure required if RN.
  • Certification in hospice preferred for RN and MSW
  • Proficient in electronic documentation and basic computer skills (Word processing, Data Entry, Outlook, Word)
  • Current California driver’s license with proof of insurance required.
