Admission Director
The Elizabeth Scott Community, a family-owned healthcare facility, is looking for a n Admission Director to join our team! We are a 5-Star Facility as rated by the Centers for Medicare and Medicaid Services.
The primary purpose of this job is to ensure the public is aware of the services provided by The Elizabeth Scott Community. This position promotes the facility in the community as well as works to increase the census, especially in Assisted Living.
Duties and Responsibilities include but are not limited to:
a) Support and promote the philosophy, goals, objectives and programs of the facility.
b) Meet with potential residents and their families for purposes of having them choose Elizabeth Scott as their care provider. Conduct tours of the facility for perspective customers as needed.
c) Respond to skilled referrals via alerts (text, email, fax, phone calls) promptly.
d) Maintain bed availability.
e) Submit possible admissions for review with Accounts Payable, DON or Clinical Coordinator.
f) Once resident is accepted for admissions, communicate with the hospital’s social worker managing their referral.
g) Conduct Pre-cert if resident has managed care if applicable.
h) Set up chart at appropriate nursing station.
i) Communicate with the hospital’s Social Services and request necessary paperwork and give instructions such as:
i) CRF (Community Referral Form)
ii) Applicable prescriptions
iii) Give detail information for transport to the facility and appropriate building and phone number for appropriate nursing station.
iv) Request HENS/7000 PASR be completed prior to discharge
j) Conduct check for all potential residents on the Sex Offender website.
k) Ensure CRF is put in chart and nurse informed.
l) Prepare admission packet with resident’s information.
m) Set up Care Conferences
n) Ensure all admission paperwork is signed and complete.
o) Ensure admission paperwork is uploaded to PCC.
p) Attend daily team meetings.
q) Serves on-call on a rotating basis for weekend or after-hours admissions.
r) Performs related duties as assigned.
Qualifications:
Education: Graduation from an accredited college or equivalent experience. Registered nurse or licensed practical nurse preferred.
Experience: At least two years’ work experience in a long-term care.
