The Patient Navigator demonstrates knowledge of SDOH, Health Equity, and Cultural awareness. They will have knowledge of medical terminology and basic medical and social screenings along with excellent telephonic and interpersonal communication skills.
They must have the ability to work in a collaborative partnership model with all members of the Interdisciplinary team, both internal and external and demonstrate a professional commitment to advocacy, initiative, integrity, flexibility and good team spirit.
Location: Far Rockaway, NY (Queens)
Schedule: Mon-Fri; 8:30a-5:00pm
Position type: Long-term contract assignment.
Hourly pay range: $20-$23hr
