Social Worker
The primary purpose of this position is to assist in implementing, evaluating, and participating in the overall operation of the social services department in accordance with current federal, state, and local standards and regulations.
Education
Most possess, as a minimum, a minimum of a bachelor's degree in social work or a human services field including sociology, gerontology, special education, rehabilitation counseling, psychology, etc.
Experience
Must have, as a minimum, one year supervised social work experience in a health care setting working directly with individuals.
Responsibilities/Duties
- Provide services needed by all residents as required by state regulations
- Handle resident and responsible party complaints and/or direct complain to proper department
- Attend Q.A. meeting daily and quarterly
- Attend care plans
- Complete charting in a timely manner - MDS, Discharge planning, Progress notes
- Advise family of community resources to care for residents needs when discharged to home
- Assist family members in gathering information needed for applying for Medicaid
- Take pictures of all new admits
- Send sympathy cards
- Send letters to families regarding bed reserve, hospital and home leave days
- Send care plan letters to responsible parties
- Schedule transportation for resident appointments
- Duties may be added/changed per Administrator
