POSITION SUMMARY
Responsible for assessing the needs of the veterans, designing a case management plan, and coordinating all activities related to services
ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential functions include but are not limited to the following:
- Assess veteran for service needs
- Develop and monitor an individualized case management plan based on the needs of the client
- Maintain proper documentation in client files
- Maintain an up-to-date roster of current clients
- Attend staff meetings as scheduled
- Work with VA liaison on veteran service eligibility, housing, and goal progress
- Work with Coalition for the Homeless to enroll clients in housing programs and/or monitor progress in housing
- Do twice weekly and random room checks to ensure cleanliness and proper condition for move in/out.
- Connect veterans with community and employment resources
- Develop and maintain relationships with veteran community providers
- Serve as liaison for clients involved in other systems such as legal and criminal justice
- Coordinate with other THP departments as needed
- Participate in all VA case conferences and technical calls
- Teach psychoeducation classes
- Provide crisis intervention as needed
- Other duties as assigned
QUALIFICATIONS / REQUIREMENTS:
- High School Diploma or Equivalency
- Case Management experience required
- Veteran Preferred
- Valid Driver's License
- Ability to work a flexible schedule
- Strong organization and time-management skills
- Must be proficient in basic computer skills, including working with Microsoft products such as
- Excel, Word, and Outlook
- Experience working with electronic health records
