```Job Overview:

We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals in need. This is a rewarding opportunity to make a positive impact on the lives of others.

Responsibilities:

- Conduct comprehensive assessments of clients' needs and develop individualized care plans

- Provide counseling and support services to clients, including crisis intervention and addiction counseling

- Collaborate with other professionals, such as social workers and healthcare providers, to coordinate services for clients

- Assist clients in accessing resources and services, such as housing assistance (Section 8), workforce development programs, and behavioral health services

- Monitor client progress and make necessary adjustments to care plans as needed

- Advocate for clients' rights and ensure they receive appropriate care and services

- Maintain accurate and up-to-date documentation of client interactions and progress

Requirements:

- Bachelor's degree in social work or a related field

- Proven experience in case management or a related field

- Knowledge of social work principles, motivational interviewing techniques, and crisis management strategies

- Familiarity with behavioral health issues and addiction counseling

- Strong communication and interpersonal skills

- Ability to work collaboratively with a diverse range of individuals and organizations

- Excellent organizational skills and attention to detail

Job Type: Full-time

Pay: From $17.50 per hour
