The Key Program is seeking a Case Manger also known as a Service Coordinator for the Family Networks Program in New Bedford, MA. The Service Coordinator will work directly with the Department of Children and Families (DCF), within the DCF regional office.

- Conducts intakes, interviews and assesses clients in preparation for referral to the network.
- Coordinates and facilitates treatment plan review meetings, initial diagnostic conferences and discharge planning meetings.
- Ensures appropriate service implementation and case coordination.
- Works with providers to ensure that client documentation, billing and data entry is accurate and timely.
