The Lutheran Home: Belle Plaine is seeking an RN Admissions Nurse to oversee our admissions and referrals department under the direction of the DON for our 75 bed skilled nursing facility.
Duties include, but not limited to:
- Respond to and follow through on all inquiries regarding placement
- Prepare and review all admission documents required by Government regulations and policies of The Lutheran Home
- Coordinate hospital and from home admissions and update admission documents.
- Provide information, promote and represent TLH through various channels including key contact sources, community events, etc. Maintain a positive relationship with the business and health care community.
- Notify other departments of admissions/readmissions. Prepare and distribute notices of admissions, discharges, new admissions or other changes for nursing home and apartments.
- Meet with and tour all individuals/families making inquiries.
- Review paperwork of potential admits to determine facilities ability to meet resident’s needs.
- Determine estimated case mix/RUG score of potential residents.
- Assist floor nurse with transcription of orders as time allows.
- Maintain a program that allows regular personal contact with active and prospective referral sources.
- Maintain a productive working relationship with the various media and establish an understanding of residents’ rights to privacy.
- Contact the news media when new programs, promotions, etc., are implemented within the facility that will be beneficial to The Lutheran Home and community alike.
- Coordinate visits to hospitals to see recuperating residents and resident homes to see discharged residents.
