Antimicrobial Stewardship Pharmacist

Job Description: EDUCATION: Doctoral Degree- PharmD required. EXPERIENCE: 3-5 years of related experience or ASHP accredited PGY1 residency required. ASHP accredited PGY2 Infectious Disease preferred. CERTIFICATE, LICENSES, REGISTRATIONS: Current Nebraska Pharmacist license required. SIDP or BCPS preferred. PRIMARY JOB DUTIES: Monitors antibiotic therapy for appropriateness and makes recommendations for change when necessary. Recommendations that improve antimicrobial therapy though streamlining therapy, encourage oral antibiotics when appropriate, broaden antimicrobial coverage when needed, and are made with infectious disease physician's input as required are given to medical personnel taking care of patients through the use of chart-based communication with verbal input as necessary. Provides drug information to pharmacists and physicians upon request. Pharmacist receives input about inquiries regarding antibiotic selection and dosing. Solicited physician requests are answered formally or informally within an appropriate timeframe. Provides educational in-services and materials for pharmacists and other health professionals as required. Pharmacist performance is enhanced through up-to-date and pertinent knowledge of antimicrobial use.
