As a Social Worker, you will be an essential part of the interdisciplinary team that makes a difference in the lives of our patients every day. You will perform a variety of medical social work services, coordinate the treatment plan with the other team members, and communicate with the patient’s physician and family. Care is provided one-on-one with in the patient’s home.

Responsibilities:
- Provides patients and families with community resources as needed.
- Acts as a consultant to the home health interdisciplinary team members as needed.
- Works with team members, patients, and families to understanding the social and emotional factors related to the health of the patient.
- Helps patients and families to understand, accept, and follow medical recommendations and provides services planned to maximize individual capabilities.
- Helps patients and families with personal and environmental difficulties that predispose illness or interfere with obtaining maximum benefits from medical care.
- Engages in resource planning by making arrangements with other supportive services for which the patient is eligible.
- Participates in discharge planning activities and prepares appropriate documentation.
Qualifications/Experience:
- Minimum of 1 year of recent experience as a Medical Social Worker.
- Board Certification by the State of Virginia
Education:
- Master's degree (MSW) from a school of social work accredited by the Council on Social Work Education.
- Geriatric social work experience preferred.
