What are the responsibilities and job description for the Registered Nurse (Intake) position at The Medical Team?

Our organization is seeking a qualified Registered Nurse (RN) - to work intake within our Home Health department to assist in the provision and coordination of services in accordance with Agency philosophy and established standards of home health and nursing practice. This position is working Monday - Friday business hours in our Corporate office.
The ideal candidate will have experience and the following qualifications:
- Prescreening and precertification of potential patients for our Home Health department.
- Manage department referral log.
- Responsible for referrals and admissions per regulations.
- Prior and/or recent certified Home Health experience, preferred
- Knowledge of Medicare Home Health Admission Criteria
- 1 year of recent Home Health experience
