Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team at The Primary Health Network. As a Community Health Worker, you will play a vital role in promoting health and wellness in our communities by providing education, support, and advocacy to individuals and families.

Key Responsibilities

• Provide culturally sensitive health education and information to patients and communities
• Assist patients with navigating the healthcare system, including scheduling appointments and resolving insurance issues
• Offer emotional support and validation to patients, as well as connect them with community resources and services
• Conduct outreach and engagement activities to promote health and wellness in underserved communities
• Collaborate with healthcare providers and community organizations to develop and implement health education programs and services

Requirements

• Bachelor's degree in a related field (e.g. public health, health education, social work)
• Certified Community Health Worker (CCHW) certification or willingness to obtain
• Valid driver's license and reliable transportation
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment and prioritize multiple tasks
