Position Overview
This person will perform duties to support grant funded projects. The position will work with a multidisciplinary team designed to support grant funded efforts.
Location and Hours of Position
The general office hours for this position is Monday-Friday 8AM-5.00PM (Must be available to work evening/weekend hours, as required)
The position is located in Sharon, PA.
Duties
- Identify patients within the health system who may be eligible for grant participation.
- Contact patients for screening, consenting, and connecting patients to grant efforts.
- Documentation within EMR system on patient progress within grant efforts.
- Email, phone communication, and collaboration with primary care providers, site leadership and staff.
- Email and phone communication with external grant partners.
- Other duties as assigned.
Education/Experience Requirements
Licensed Practical Nurse or Medical Assistant
OR
Bachelor's degree in related field
