Schedule: FT/E, Varies
Supervises: Lead Hosing Navigators, Case Managers, Housing Navigators

Rate of Pay: $61,000-$65,000

Benefits: Standard; Full-Time, Exempt employees are eligible for but not limited to the following:

Function: The Housing Program Manager provides leadership to the housing focused case management services program for the homeless men residing at the Crossroads Resource Center. Supervision of all case managers, housing navigators and interns at Crossroads. This includes providing oversight for assessments, individual housing plans, and housing first program design for guests to successfully become rehoused in the broader community. The Housing Program Manager works with the Lead Housing Navigator to develop housing first programming for homeless clients that is client-centered and trauma-informed. The Housing Program Manager also coordinates with the Shelter Program Manager to identify creative interventions to address issues with shelter clients.

Duties and Responsibilities: (List by section/topic as possible)

  1. Oversee the provision of case management services, housing navigation services, and staff/interns
  2. Assess and develop a best practice re-integration program and case management plan for residents
  3. Develop employment training assistance for residents
  4. Network with and connect appropriate community resources/services

Scope of Position/Essential Functions:

A. Manage and lead staff to create a positive environment for staff and guests.

  1. Lead and manage teams: hire, train, cross-train and develop program staff for all case manager and housing navigators. Develop individual goals and self-care plans, provide ongoing feedback and one-on-one meetings, and conduct performance evaluations.
  2. Provide supervision and support to Program Director while leading the coordination efforts with city and community partners on the housing/case management team.
  3. Manages HR related issues including hiring, staff discipline and conflict resolution in a timely manner.
  4. Train staff to be proficient in responding to both simple and complex situations while using trauma informed care and appropriate crisis intervention strategies.
  5. Ensure staff complete all required onboarding trainings.
  6. Ensure appropriate staffing and productivity by program staff; coordinate work assignments, workflows and ensure regular staff meetings.
  7. Oversees staff schedules while ensuring appropriate staffing levels and low overtime utilization.

B. Oversee Case Management Services and Staff.

  1. Implement and oversee case management services that effectively transitions men to greater self-sufficiency
  2. Create a plan for utilizing internships within the case management program
  3. Supervise case management staff and interns within the program
  4. Establish standardized assessments and housing plans for each person within the case management program
  5. Keep up-to-date documentation and records of all men participating in case management programming in appropriate databases.
  6. Produce timely data reports, including outcome measures and quarterly reports for city contracts.
  7. Monitor and track financials related to case management and housing services.
  8. Complete the following training: The Salvation Army Driver Training, The Salvation Army's Protecting the Mission Training, The Salvation Army Pathway of Hope Training, HIPPA Compliance Training, Clarity and WellSky databases.
  9. Must be at least 21 years of age and possess a valid CO driver's license.

C. Develop a Housing Focused Case Management Program.

  1. Assess need and potential impact of a comprehensive re-integration program for single homeless men
  2. Create a case management program for Crossroads guests that assists and encourages them to work toward greater self-sufficiency.
  3. Assist in establishing a basic data analysis of program effectiveness and impact relative to outcome objectives and measures.
  4. Develop outreach and recruitment strategy to engage guests in case management programming.

D. Housing First Employment Training Assistance

  1. Establish multiple and diverse employment training opportunities for program participants to reduce barriers to housing.
  2. Secure external career assessment and guidance services
  3. Promote community partnerships for permanent employment for residents

E. Network with Community Resources and Partners

  1. Initiate networking relationships with appropriate community and service agencies.
  2. Encourage coordinated access/connection for residents with necessary community services
  3. Participate in community meetings and collaborations as assigned by the Shelter Administrator
  4. Promote community and for-profit involvement with support
  5. Referrals as needed.

F. Special Projects

  1. Offer leadership to special projects assigned by the Program Director.

Knowledge, Skills and Abilities Required:

Commitment to the mission of The Salvation Army

Strong communication and people skills

Demonstrated history of providing professional, effective case management services

Competent in providing a positive, solution-focused approach to working with troubled program participants

Able to conceptualize, teach and model case management skills, including motivational interviewing and trauma informed care.

Experience providing supervision to staff.

Desire to serve others by building on their strengths

Competent in working independently within clear parameters in a team environment

Qualification and Education Requirements:

Master's in social work (M.S.W.) with at least 2 years, postgraduate, professional social service experience (L.C.S.W. preferred), preferred; or master's degree in social services field preferred. Currently credentialed to provide supervision to M.S.W. candidates according to the State of Colorado, preferred. A Bachelor's degree in a mental health field with at least 3 years case management experience working with individuals with chronic/acute health, mental health needs will be considered.

Track record in working with people requiring critical case management experience. 1 year experience working with unhoused populations required. Experience with addiction recovery populations is preferred.
