The Oasis Case Manager is primarily responsible for ensuring effective management of youth’s individual service plans during their stay at the YMCA Oasis Teen Shelter, a 9-bed, state-licensed Overnight Youth Shelter (OYS) for youth ages 13-17 in Mount Vernon, WA. This position reports directly to the Shelter Manager, working collaboratively with the whole Oasis team and across the Y’s Social Impact services to ensure coordination of youth’s care. The Oasis Case Manager assists with ensuring the safety and wellbeing of at-risk and homeless youth while in shelter. This position requires modeling of pro-social, healthy adult behavior and decision-making to adolescent clients. The Oasis Case Manager delivers affirming, culturally competent, trauma-informed care in pursuit of a more stable housing outcome as youth exit the program.
This position is for you, if you can:
- Model and execute a non-rejecting, trauma-informed care approach to working with at-risk and unstably housed adolescents. Ensures a positive, growth-enhancing environment.
- Build high-quality relationships with youth participants to ensure effective care and stable housing outcomes while youth reside in a temporary overnight shelter setting.
- Provide individualized case management and coordination of services:
- Conduct assessment with each youth to determine goals, barriers to goals, resource needs and other factors related to service planning, with particular attention paid to the following: housing (per housing first model), education, job training, menta/physical health, substance use and social supports
- Develop an Individualized Service Plan (ISP) for each youth based on assessment of their needs and hold youth accountable to expectations of the program
- Engage with additional community resources and systems of care to effectively serve youth and widen their network of support
- Coordinate safety planning and crisis response of youth when needed
- Provide follow-up and aftercare services as needed
- Provide or coordinate necessary transportation for youth as needed
- Coordinate with the Shelter Manager, Shift Supervisors, Youth Advocates, and Executive Director of Social Impact on youth entry and exit of shelter, length of stay, disciplinary concerns and other coordination of care
- Member of the on-call administrator pool
You’ve got this, if you are / or have:
- Bachelor’s Degree Required in social services or closely related field from an accredited school. Master’s degree in Social Work or closely related field preferred.
- Two years or more of related experience working with homeless, at-risk, and/or other youth populations.
- Strong skills in self-initiation, communication, collaboration, crisis management and behavior management.
- Ability to work variable hours, such as late nights or weekends
- Must be able to pass a criminal background check from DCYF and the State of Washington
- State approved certifications in CPR/AED, First Aid & Bloodborne Pathogens, as well as Mandatory Reporting and Trauma-informed Care/Harm Reduction training required within 90 days of hire.
- Requires negative TB Test before working within facility
Qualifications
Preferred Qualifications:
- Experience with school systems (special education, 504 service plans, IEP’s, etc.)
- Experience case managing clients within other systems of care, such as behavioral health, child welfare, and/or other housing providers
- Previous training in de-escalation, Positive Youth Development, CSEC, Domestic Violence, Mental Health First Aid, or other related trainings.
