A health services network in New York is looking to add a licensed Registered Nurse (RN) to join their Hospice Home Care team in Rockland County.

- Provide optimum patient care in an efficient and cost-effective manner
- Assure the delivery of safe, therapeutic patient care, including supervision of paraprofessionals providing services to patient according to regulations
- Assess patient care needs and intervenes to ensure that needs are met in an appropriate and timely manner: provide symptom management and specialized nursing skills related to end of life care
- Coordinate the daily care of the patient: inform the Physician and other personnel of changes in patients’ needs and outcome of interventions
- Develops and maintains effective relationship with patients and families; contribute to patient and family counseling and education regarding disease process, self care techniques, prevention strategies, end of life care process and dealing with ethical issues
- Contribute to patient and family counseling and education
- Participate in QAPI program and hospice sponsored in-service training in compliance with professional, regulatory and agency standards
- Participate in ongoing interdisciplinary comprehensive assessment
- Perform other duties, as needed
- 1 year of recent Bedside Nursing experience
- Bachelor's Degree in Nursing
- NYS Registered Nurse (RN) license
- Comfortable providing Community-based Care
- Experience caring for terminally ill patients
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
