Job Description

Summary/Objective:

The Social Services Coordinator/Assistant Administrator has the responsibility of completing all documentation in compliance with applicable state, federal and local regulations and in accordance with the policies and procedures set forth by The Voyage Senior Living. They will serve as an advocate for all residents while working directly with the Administrator and Facility Manager to establish and adjust care plans that are designed to preserve and maximize the independence and dignity of all residents. They will develop daily activities that will allow residents at all functioning levels to participate. They will assist the Marketing Coordinator as needed in promoting their facilities.

· Understands all federal laws, IHDA and HFS requirements governing certification and leasing operations (e.g. resident selection and marketing plans) and assures that all requirements of the LIHTC program are implemented. Understand and keep up-to-date on all documentation needed for admissions of residents.

· Completes Income Certification Forms and collects all supportive documentation upon admission and on an annual basis. Works cooperatively with residents, family members and guardians to obtain proper documentation as needed.

· Understands all requirements of the Illinois Department on Aging Community Based Residential Facility Program and assures compliance with all policies, procedures and regulations.

· Understands eligibility criteria of the Community Care Program, makes appropriate referrals to CCUs and screens potential residents who may qualify for the IDOA program.

· Assess and develop Care Plans for all residents, requests a re-evaluation from the CCU when a resident’s status has changed and re-assess the residents on an annual basis or when there has been a significant change of status.

· Completes a social history on each resident based on information provided by the resident, family or guardian. Reviews and updates social history monthly.

· Orients new residents to the building. Provides all residents copies of pertinent documents, including but not limited to: resident agreements, resident rights and responsibilities, resident plan of care, grievance procedures, schedules of activity programs, emergency procedures, etc.

· Meets with individuals, family members and others demonstrating interest in The Voyage Senior Living facilities. Provides tours, distributes written information to interested parties and completes the lease documents.

· Coordinates monthly activity calendars that will include activities allowing residents at all functioning levels to participate.

· Assists the Administrator develop and implement Negotiated Risk Agreements with residents, family members, guardians or power of attorneys as needed. Works with the Administrator and their duties to be able to serve as a backup when necessary.

· Coordinates and attends all resident’s monthly meetings.

· Manages and responds to resident incident reports and follows up appropriately.

· Works with residents and appropriate family or guardian to develop a suitable plan of discharge when assisted living is no longer able to meet the resident’s needs.

· May be required to attend seminars, workshops and organizations to enhance skills and network with others

· Perform all other duties as assigned

Physical Demands and Expectations:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

· The employee is regularly required to talk, hear and have the ability to follow written and oral instructions and procedures

· The position requires frequent sitting, walking and standing, occasional bending, stooping, kneeling, twisting, stretching/reaching and climbing stairs. Requires occasional lifting and carrying up to 15 pounds.

· Must have dependable transportation, a valid Illinois Driver’s License and State required liability insurance on a personal vehicle

· May be required to work a flexible schedule and be willing to attend events after hours or assist in facilities occasionally

· Must be willing to attend in-service training, seminars and other activities as assigned

· Appropriate documentation indicating absence of infectious/communicable diseases, e.g. TB and Hepatitis may be required

· Position is subject to a criminal background check

Education and Experience:

· Bachelor’s Degree in marketing or health-related field preferred or at least 3 years of experience in a related field

· A minimum of 2 years of experience working with the elderly preferred

Pay: $38,000.00 - $43,000.00 per year
