The W Assisted Living Communities is is seeking a compassionate and dedicated Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with a positive and supportive team of healthcare professionals.
Job Responsibilities:
- Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
- To communicate with the resident’s care providers
- Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
- Coordinate and supervise clinical care for residents
- Maintain clear and concise documentation in accordance with facility requirements
Qualifications/Requirements:
- Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required
- Previous Compuware experience is a plus
- Excellent phone skills and ability to work with families/other care providers
- Strong organizational skills, passion and compassion for the population
- Ability to work cooperatively with multidisciplinary team
