REACH serves as the community quarterback for comprehensive community revitalization in the Riverside neighborhood, located in northeast Wilmington, DE.  REACH supports and leverages efforts and resources which promote upward economic mobility for Riverside residents. Employing the Purpose-Built Communities’ model, the revitalization focuses on three pillars: 1) redevelopment including high-quality, mixed-income housing, 2) a cradle to college and career readiness pipeline, and 3) community health, wellness, and safety initiatives. REACH, in collaboration with service partners Kingswood Community Center and The Warehouse (an innovative teen center with more than 150 partner organizations) work together to transform Riverside into a robust, healthy, and vibrant community. The Warehouse, REACH Riverside and Kingswood Community Center are collectively known as the "WRK Group".
 
Kingswood Community Center Overview:
Founded in 1946, Kingswood Community Center, Inc. (KCC) sits in the heart of the Riverside Community located in Northeast Wilmington. KCC's mission is to enable people in Northeast Wilmington and surrounding communities to achieve their potential for economic, social, and personal well-being. KCC offers programs for the youngest and oldest alike, as it includes an early learning center for 12 months to 5 years of age, before and aftercare, after school programming for school age and teens, and a senior center for the elders in the community. In addition to programming, KCC provides resources, opportunities, and events to all ages.
 
Position Summary
The Navigation Coach is responsible for building partnerships with families to assist them in identifying strengths, needed services, goals, and support systems in order to facilitate economic mobility.  The Navigation Coach works as a part of the interdisciplinary team of the EMPOWER (Economic Mobility Places Ownership Within Everyone’s Reach) Program, advocating for participant needs and creating a strategy for holistic services.  This position is also responsible for increasing the availability and accessibility of a range of services for high-risk families by coordinating partnerships with organizations to provide health, financial, and educational, and crisis alleviation programs and services. 
 
This opportunity is ideally suited to an experienced case/care management professional who is high-energy, innovative, collaborative, able to move “at the speed of WRK” (fast paced) and interested in using skills to help evolve the way we serve our customers.
 
Essential Functions
 
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These essential functions are performed on a frequent and recurring basis by an incumbent and including but not limited to:
 
  • Develop and lead new and expanded initiatives in a collaborative, fast-paced environment.  Devise strategies to ensure growth of programs and initiatives.
  • Provide case management and economic mobility services for participants to reduce or alleviate barriers to active participation and maintain continuity in services.
  • Assist with recruitment, including promotion of the program to interested families and assistance to families in obtaining needed documents for enrollment (i.e. proof of income, ID). 
  • Conduct orientations, collect intake materials, and screen participants for eligibility.  Administer, score, and schedule related assessments.
  • Upon enrollment, develop an individualized plan that responds to the family’s need.  Incorporate the family’s goals, strengths, responsibilities and timetables and strategies for achieving these goals.  Provide direct service and/or coordinate needed services for each family, reviewing and updating the plan at least quarterly. 
  • Maintain regular contact with every family.  Work with families to reduce barriers to participation in programs and activities.  Conduct outreach for families who need to be reengaged.
  • Create, review, manage, and update files for each family, ensuring files are up-to-date and organized per grant requirements.  Conduct periodic file reviews to ensure compliance. Enter data into the CRM system. 
  • Implement program evaluations and assessments (i.e. surveys, environmental scans) to include program delivery, customer service, and effectiveness and progress toward program objectives.
  • Assist with the coordination and implementation of events (such as recognition and incentive activities, job fairs, community events, and participant orientations).
  • Uphold agency policies and standards, ensuring legislative regulations are followed (e.g. accidents, incidents, and disciplinary actions) and compliance with contractual requirements.
  • Analyze information and maintain complete, organized, and accurate documentation (including email communication). Prepare and submit timely reports and other documentation.
  • Network with businesses, nonprofits, schools, and government offices to identify resources and develop and maintain partnerships to strengthen service delivery, to develop programs, and address community needs.
  • Represent the organization in public forums and with donor agencies and foundations as required.  Promote awareness of the agency and its mission in the community.
  • Maintain an active understanding of agency programs in order to increase opportunities for support and collaboration.
  • Practice continuous learning through individual study, classroom training, seminars, and conferences.
  • Participate in regular meetings/discussions (i.e., staff, Multidisciplinary Team and WRK Group meetings), taking meeting minutes as needed.

Supervisory Responsibility
This position does not have supervisory responsibility. 
 
Position Type and Expected Hours of Work 
This is a full-time position, and general hours of work and days are Monday through Friday, 9:00 a.m. to 5 p.m.; however, this position regularly requires after-hours and weekend work.
 
Travel
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
 
Education and Experience 
  • Bachelor’s degree from an accredited college or university in Social Services, Human Services, Early Childhood Education, or related field OR Associate’s Degree and ten years of equivalent experience.
  • At least two years of experience working with at-risk participants in a social services or education setting. 
  • Bilingual in Spanish or Arabic (preferred)
Certification Requirements
  • First Aid/CPR preferred; must be certified within first 60 days of hire.
 
Conditions of Hire
  • A satisfactory criminal background and reference check, a valid driver’s license, and an acceptable driving record are required at time of appointment and throughout employment in this position.
  • Must be eligible to work in the U.S. without sponsorship.

Work is performed in an office, classroom, and in the community (such as schools, businesses, parks/playgrounds). Office and classroom work require sitting for periods of time and the use of a computer, keyboard, and screen, as well as teaching tools (whiteboard, projector, etc.). The incumbent sits, stands, walks, kneels, crouches, twists, climbs stairs and inclines, reaches, bends, grasps, pushes, pulls, and drags. Office/classroom work is often performed with moderate noise levels and requires the ability to focus.  The incumbent must be able to move about to facilitate workshops, access files, operate equipment and office machinery, and lift and/or move up to 20 pounds. Work performed in the community requires the incumbent to be subject to different environmental conditions, including outdoor conditions, extreme heat and/or cold, and loud noise level.  Outdoor work may require periods of walking, standing, stooping, crouching, kneeling, reaching, pushing, pulling, and lifting, as well as peripheral vision, distance vision, depth perception, and the ability to focus.  If driving a vehicle on organization business, the incumbent must be able to meet the physical requirements of the driving class and have mobility, vision, hearing, and dexterity levels appropriate to the duties to be performed.  
 
Equipment Use: Telephone, copier/scanner, computer/laptop (Microsoft Office Applications), fax machine, tablet, and projector.
 
Mental demands: Solve problems, make decisions, exercise sound judgment, supervise, interpret data, organize, write, plan, prioritize, communicate clearly, de-escalate situations, model respectful non-abusive behavior, critical thinking, critical reasoning, attention to detail, ability to recall, coordinate schedules, work as a member of a team, and speak in public.
 
Evaluation of Performance: Performance in this position will be evaluated in accordance with the Personnel Practices Manual.
