Home Health RN

Our Home Health Agency is looking for a Registered Nurse to join our team and serve members of the community who suffer from Chronic Kidney Disease.

This is a Full Time, Salaried position.

You will travel to patients homes, and be compensated for travel time.

Schedule:

• We generally operate Monday - Fridays, 8am - 5pm
• There will always be a chance that a patient requests a visit after 5pm; you should be flexible in this way
• You will be asked to cover a 1 Hour Radius; that means you could be driving up to 1 hour to a patient's home. You will be considered on the clock while driving, and thus will have days where you only see 1 - 2 patients in your 8 hour shift.
• Most days you will see 3 - 4 patients
• 80% of the job is on the road visiting patients.

Job Description:

• Deliver evidence-based, timely care in a manner that reduces hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
• Conducts assessments, to include comprehensive annual wellness exams
• Prescribe medications, order tests, and collaborate with physicians
• Collaborate with care management RNs and SWs to ensure consistent highly satisfying patient experience, accurate documentation, and effective care planning
• Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily

Job Qualifications:

• Valid RN License
• 2 Years RN Experience
• 2 Years of Home Health, Hospice, or Dialysis Experience
• Must Be Fully Vaccinated

Preferred:

• Bilingual in English and Spanish
