LPN Hospice Nurse Position

• Location: White River Junction, VT 05001
• Shift: Day shift - 8:00am - 4:30pm, Monday - Friday; weekend on-call days rotation
• Number of weeks: 16
• Guaranteed Hour: 32 hour/week
• Overtime: Yes, start after 40 hour/week

Minimum Qualifications:

• Must be a Licensed Practical Nurse in Vermont and New Hampshire.

Responsibilities:
- Provide holistic nursing care to terminally ill patients in hospice settings.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments following physician orders.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Offer emotional support and guidance to patients and their families.
- Ensure compliance with HIPAA regulations and maintain patient confidentiality.
- Monitor patient conditions closely and provide symptom management.
- Educate patients and families on end-of-life care options.

Qualifications:
- Degree in Nursing (RN) from an accredited program.
- Active RN license in [State] or another U.S. state.
- Experience in hospice or palliative care is preferred.
- Proficiency in basic life support (BLS) and knowledge of ICU protocols.
- Familiarity with ICD-10 coding and medical office procedures.
- Ability to phlebotomize for lab tests.

Skills:
- Strong diagnostic evaluation skills.
- Experience in home community care or primary care settings.
- Proficient in basic life support techniques.
- Knowledge of HIPAA regulations and patient privacy laws.
- Familiarity with ICU practices and protocols.

Nice-to-Have Skills:
- Previous experience in toddler care or pediatric nursing.
- Exposure to working in critical care environments like ICU or Level II Trauma Centers.
