Job Summary

We are seeking an organized, motivated, compassionate individual to provide navigation through our Deep Brain Stimulation (DBS) program, a multi-disciplinary program devoted to the surgical treatment of Parkinsonrsquo;s Disease and related movement disorders. This position requires strong interdisciplinary skills, and the desire to develop and coordinate pathways, workflows, and relationships between patients and care partners, physicians in Neurology, Neurosurgery, and other subspecialists involved in the DBS program. The existing Neurological and Neurosurgical teams already have specialized physicians, nurses, physician assistants and other providers administering direct treatment and care. The navigatorrsquo;s role will differ in that the primary role is to ensure seamless coordination to improve the patient experience, eliminate unnecessary delays, provide education to patients and the community, and to help monitor and evaluate the programs outcomes and effectiveness.

Essential Functions

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Provides specialized nursing guidance, care, and education to patients in accordance with physician/nursing orders and established policies and procedures. Serves as a resource for the patient and family regarding the preoperative evaluation, inpatient procedure, hospitalization, and the perioperative period.

Patient Navigation through the Deep Brain Stimulation (DBS) pathway:

  • Demonstrates an ability to assess patient needs and make appropriate referrals to provide navigation and guidance to Movement Disorders patients through the Deep Brain Stimulation (DBS) pathway.
  • Navigation services may include addressing barriers, resource referral, and care coordination.
  • Utilizes and identifies available resources to assist patients with overcoming barriers to receiving appropriate access to care and clinical trials.
  • Employs seamless processes for patient referrals to navigation services across the DBS continuum.
  • Facilitates visits necessary for preoperative evaluation including with neurosurgery, neurology, neuropsychology and radiology in patients undergoing DBS surgery.
  • Facilitates visits necessary for postoperative care in patients undergoing DBS surgery, including with neurosurgery and neurology.

Education and Research:

  • Coordinates the development, utilization, and evaluation of appropriate educational resources and interventions that assist the patient/caregiver in developing the skills for self-care.
  • Assists in the coordination and implementation of DBS educational workshops to inform the community of resources offered by the Movement Disorder Center.
  • Assesses the educational needs of the patient/caregiver as they relate to the DBS process and needs across the perioperative time course.
  • Utilizes clinical research to support evidence-based clinical decision making and practice.

Organization and Logistics:

  • Develops strategies to improve or maintain the effectiveness of the Movement Disorder Centerrsquo;s DBS program.
  • Schedules and leads the monthly interdisciplinary-multidisciplinary meetings.
  • Keeps current with advances in the scientific area, considers the impact on the DBS program, and oversees necessary implementation.
  • Uses advanced subject matter expertise in the therapeutic area or clinical research area to solve complex problems or foster innovation.
  • Maintains a database that captures outcomes, how patients were referred, the types of barriers patients faced, how the patient interacted with our team, who provided the service and how the multidisciplinary team was informed of the patient#39;s needs.
  • Oversees implementation of data quality processes, data flow plans, and new technologies.
  • Participates in quality/performance improvement programs, product evaluation, outcomes measurement, and/or clinical research.
  • Manages progress of collaborating stakeholders toward achieving project/program goals.

Communication:

  • Establishes and maintains internal and external communications to ensure successful partnerships.
  • Communicates effectively with others, regardless of reporting relationship, to accomplish shared work objectives.

  • Assists in facilitating delivery of tissues and managing documents/data.
  • Participates in the organization, development, production, and presentation of programs locally and regionally

Education: Bachelorrsquo;s degree in Nursing required
